Lakeland Regional Health (LRH), located in Lakeland between Tampa and Orlando, is Florida’s fifth largest hospital with 849 beds. It is a community not-for-profit regional referral center with the busiest single-site Emergency Department in the nation. LRH is a Joint Commission accredited chest pain, primary stroke, and level II trauma center.
The Clinical Pharmacist works within the multidisciplinary team to provide care to ICU and Intermediate Care Unit (IMCU) patients. The pharmacist works weekends (Saturday and Sunday 7:00 am to 7:30 pm) plus one evening a week (2:30 pm to 11:00 pm). The responsibilities include covering two of the four units including order verification, clinical, and rounding with the physician-led multidisciplinary team. The three proper ICUs include 18 beds each (Trauma, Surgical, Medical) and one IMCU (29 beds covered by the Critical Care or Trauma Service).
Provides comprehensive clinical pharmaceutical care and actively participates in team development.
Assess therapeutic appropriateness, provides pharmacokinetic dosing and monitoring, nutrition support, drug information, and patient population specific dosing with consideration for age, renal and hepatic function, microbiology lab and culture results, and underlying conditions.
Completes all documentation in the patient chart and monitoring tools for subsequent pharmacists.
Reviews and verifies electronic orders via CPOE and contacts the patient’s nurse or physician/healthcare provider to ensure appropriateness, completeness, clarity and safety of each order when needed.
